*Patients previously diagnosed with periodontal disease must enroll in the periodontal membership plan.**

- No waiting period for coverage
- No deductible or pre-authorizations needed
- 20% discount offered with each plan applies to restorative and cosmetic procedures
*Treatment must occur within year of enroll and is non transferable to another party or family member. The enrollment fee must be paid in full and is non-refundable. Enrollment is for 12 consecutive months.*
